The word "incontinence" caught my eye in your posting. You asked about pelvic pain, but you didn't say that your client had any problem with bowel or bladder control; is that the case? The reason I ask is that the term incontinence refers to a lack of bowel &/or bladder control. If this IS the case, I would ask the client to have his physician check out the situation & find out what the problem is. [Unfortunately, many people put up with urinary incontinence & wear a Depends, when all that was wrong could be easily corrected with specific exercises, medication, or in some cases surgery. Bowel control problems are usually addressed rather quickly by the client for obvious reasons!]
If the client is NOT incontinent, they should still be checked out by their physician for any number of possible conditions -- Pelvic Inflammatory Disease, fibroid tumors (for females), STD's, bladder/kidney disorders, etc., etc. More than likely, the problem IS muscular, but it would be a shame to assume it was & find out later that another, very serious condition was to blame for the pelvic pain the client is having. Not knowing the signs & symptoms the client is presenting, I don't know if I'm "hitting the mark" about the possibility of their pelvic pain, but it's worth a close look before treating it with massage.
